A new reduced-premium, individual health coverage plan with more in-network provider choices
Health Check Basic includes:
¨ Four annual deductible options: $500, $1,000, $2,500, and $5,000 ¨ Coinsurance (after deductible): 80 percent for services in the BluePreferred PPO network; 70 percent for Blue Choice PPO Network services; 60 percent for Blue Traditional Network services; and 50 percent of the Blue Choice PPO Network maximum allowable amount for out-of-network services ¨ Prescription drug coverage: not subject to deductible; pays 50 percent of allowable charge at network pharmacies ¨ Annual stop-loss: $10,000 ¨ Lifetime Maximum: $5 million ¨ Inpatient hospital deductible: $500 for each hospital admission ¨ Outpatient surgery deductible: $200 for each visit to a hospital outpatient department or ambulatory surgical facility for surgery ¨ Emergency room deductible $100 for each visit to a hospital emergency room (waived if patient is admitted to the hospital through the emergency room visit) ¨ Office visits: You pay only $35 copayment for most doctor office visits. |
